Subject:        Patient falls

    

 

Request:

Under the Freedom of Information Act 2000, please could you provide me with the following information relating to the Royal Bolton Hospital:

1. The number of patient falls that occurred in hospital in November 2016
2. The number of these falls that had a pre-falls multifactorial risk assessment
3. The factors that are assessed in a pre-falls multifactorial risk assessment
4. Please could you categorise the number of falls by outcome in terms of harm severity?
5. Please could you categorise the number of falls by ethnic group?
6. Please could you categorise the number of falls by age?
7. Please could you categorise the number of falls by time of day?

The trust Falls prevention policy states that all staff caring for patients must ensure risk assessments and management plans are undertaken as required and that follow up preventative and management action is implemented.

To be able to answer this question fully, it would require each patient file recorded as a fall in November to be audited which would take significant time to establish.

Refusing under section 12 because will exceed the cost limit.

3. The factors that are assessed in a pre-falls multifactorial risk assessment

4. Please could you categorise the number of falls by outcome in terms of harm severity? One of the Trust’s main priorities is falls prevention, and is something that all staff work hard to achieve. In November unfortunately there was one patient fall of the most serious grading, which was fully investigated. Steps have been taken to ensure that all staff are fully aware of our policies around preventing falls in our most vulnerable patients, in order to reduce the possibility of patient harm.
