Response:

1. In reference to the attached requests relating to inpatients – we do not code cause of death. Therefore to extract the necessary information – would require a manual case note review by the responsible clinician and this will take more than 2.5 days, and would exceed the time frame to reply to the request. We are refusing the request under the ground cost.

 

2. The questions related to the incidents at home – The recording of where an incident happened is by use of a 4th character. Accident in the home includes outside of the home i.e. driveway, garden, garage, private swimming pool etc. Therefore the use of the 4th character is not always meaning within the home.

 

Accident in the home is still a generic term – which  we are unable to assign a code to. The coder would need the exact details of what/where how the accident happened. We do not hold this information.
