As a vital link and provider of service information within each NHS Hospital Trust we are approaching all UK NHS FOI Services for your valuable assistance.
We are a joint collaboration between the University of Birmingham and the University of Sheffield that has been funded to undertake a national survey to identify current care pathways in the UK for women who sustain obstetric anal sphincter injuries (OASIS) during childbirth.
The information you provide will be used to identify the services that NHS Trusts in the UK are currently offering for women who sustain OASIS.

Some of the information may need collecting from specific Obstetric Consultants within your hospital, so a hard copy of the questionnaire is attached to make it easier for the answers to be recorded and then transferred to the online survey.
