Subject:     Emergency readmission rates  

    

 

Request:

Last year, we published a briefing to look at the emergency readmission rates across 72 hospital trusts between 2012/13 and 2016/17. We are looking to bring the dataset up to date.

As you will be aware, emergency readmissions are currently recorded as any patient subject to an unplanned re-admittance within 30 days of being discharged. But to understand any impact fully this data needs to be broken down by each day within the 30 day period.

Under the Freedom of Information Act 2000 please provide the data to fill out the table below.

Secondly, please answer the following question:

How is the data collected on emergency readmissions used or analysed to i) prevent emergency readmissions; or ii) improve patient experience?

Secondly, please answer the following question:

How is the data collected on emergency readmissions used or analysed to i) prevent emergency readmissions; or ii) improve patient experience?

 

The information on readmissions, as opposed to re attendances, is collected and reported monthly, this will be across the whole organisation.

 

There have been local speciality audits and the numbers are discussed at speciality governance, we are looking to review the information in our harmfree care panels.

 

If the readmission is incident reported or there is a complaint this is dealt with on an individual basis.
