Subject:        End-of-life payments

 

 

Request:

The percentage of patients who have died on the Trust’s end-of-life care plan against the Trust’s target for the number of patients who should die on this plan over the last three years, divided by the year.

  • E.g. Between 1 January 2016 and 31 December 2016, 43 per cent of patients died on the plan against a target of 35 per cent.
  • Note: If no such target exists, please make this clear.

The amount received by the Trust, via the Commissioning for Quality and Innovation (CQUIN) system or similar, for hitting targets related to its end-of-life care plan for the last three years, divided by the year.

  • E.g. Between 1 January 2016 and 31 December 2016, the Trust received £X via CQUIN for achieving goals involving its ICP.
  • Note: If no such payments were made, please make this clear.

 

Response:

We do not have a target/percentage but would expect all expected deaths to have an individualised plan of plan of care.

End of life has not been included in any CQUIN schemes 2016 onwards therefore no payments received relating to this.
