Bolton NHS Foundation Trust’s Acute Medicine Team finalists for three HTN Awards

The awards celebrate digital teams, innovations, programmes and health tech suppliers who have made a contribution to improving health and care.

Bolton NHS Foundation Trust’s Acute Medicine Team with Allscripts are the finalists for Efficiency Savings of the Year, Health Tech Team of the year and Innovation in Digital Pathways for their Acute Medicine Referral List. The Acute Medicine Referral List is an electronic patient list that automates referrals to Acute Medicine from multiple admission streams. This referral list has been shown to ensure patients are seen quicker by a doctor, as well as reduce the total amount of time spent in hospital.

The list helps to identify and track referrals to the Acute Medicine Team, allowing the team to see which patients were still waiting for clinical assessments. It then tracks these patients as they move through the hospital via live updates. Within the list, a variety of information can be found including, patient demographics, current location within the hospital, indicators of current severity of illness and current status of clinical assessments.
