As a professional you may come into contact with children aged 0-5 and their families who you feel would benefit from additional support from our integrated health and wellbeing service.
Through our eight health visiting teams and a central infant feeding support and health improvement team we deliver 4 levels of service:
- Community – this includes peer support groups and drop in clinics
- Universal – mandated contacts offered to all children at set points in the first 5 years of a child’s life
- Universal Plus – additional support using evidence based interventions offered to families in which a specific health and wellbeing need has been identified.
- Universal Partnership Plus – ongoing collaborative support for families with complex health and wellbeing needs through a multiagency approach
Practitioners working within the integrated health and wellbeing service for 0-5s fully embrace the use of Bolton’s Early Help framework to facilitate partnership working with a wide variety of other services.
