Admission Avoidance and Community Therapy Hub

Our Admission Avoidance and Community Therapy Hub service provides urgent one to two hour assessments and support for patients over 18 in a crisis, to prevent unnecessary hospital admissions.

This can include short-term treatment using intravenous antibiotics (administered through a vein) and fluids if necessary.

Call ‘999’ now if you have signs/symptoms of a heart attack; stroke; severe difficulty in breathing; heavy bleeding; severe injuries; seizures or sudden rapid swelling of the tongue; eyes; mouth or throat.

Any urgent conditions outside of this list, please speak with your GP or contact NHS 111 for medical advice.

 

The team provides urgent assessment and intervention to Bolton residents over the age of 18 if at risk of admission to hospital.

The aim is to promote independence, safety and confidence during an acute period of illness and reduce avoidable admissions to hospital.

The team includes:
