How to access the service

Health trainer with patientPatients in need of lifestyle support are identified by their GP or practice nurse and referred directly to the health improvement practitioner based at the surgery.

Patients identified through a hospital health promotion assessment are referred to the health improvement practitioner based at their GP surgery.

When arranging appointments, we ask patients when and where they would like to be seen. They can choose to meet with their health improvement practitioner at home, at their workplace, at the GP surgery or at
another venue of their choice, and at a time that
suits them.
