Community IV Therapy

Service summary

The IV Therapy team provide domiciliary visits as well as clinic services across Bolton. ​​​​​​​

The IV team has played a vital role in delivering blood transfusions and home subcutaneous chemotherapy, providing thousands of safe treatments directly to patients’ homes.

This service, is highly valued by patients and their families and stands out as one of the few, if not the only, home transfusion services of its kind nationally.

Domiciliary services

We provide a range of domiciliary services which include intravenous antibiotics at home (including the administration of Elastomeric devices when approved by Microbiology), blood monitoring for IV Antibiotic and Haematology patients, blood/platelet transfusions, sub-cutaneous chemotherapy and Zoledronic acid.

Criteria for intravenous antibiotics at home:​​​​​​​

  • If a patient has been assessed as suitable for OPAT, then the patient must be discussed with the Consultant Microbiologist, unless you are ringing from an assessment ward and following the cellulitis pathway.
  • The patient must have a Bolton postcode and Bolton GP.
  • All referrals must be discussed with the IV Team coordinator.

Please note, that for hospital patients, the referral form is on EPR. This must be discussed with the IV Team prior to being completed, and updates must be provided to the team regarding discharge date.

Clinic services

Venesections

Patients are referred via Dr Nayeemuddin/Gastroenterology for Venesection (bloodletting), and frequent monitoring of their Ferritin and Hb for management of Haemochromatosis.

Ferrinject

Must always be administered within the clinic environment. Ferrinject is not licensed for administration in the home environment. Please refer to the factsheet for detailed information. Saved in the file – it’s a pharmacy guideline so it’s just where it links from.

Zoledronic Acid

We administer Zoledronic acid for rheumatology and The Christie’s patients. We can administer Zoledronic acid at patient’s homes for housebound rheumatology patients.

Rituximab/Tocilizumab

Patients referred from Rheumatology will attend Breightmet Health Centre for their infusions.

Infliximab/Vedolizumab

Patients referred from the IBD team will attend Breightmet Health Centre for their infusions.

How to refer into this service

Referral form

The referral form for hospital patients can be found on EPR, but please contact the IV Team prior to completing as we are not live on EPR.

If a member of staff does not answer, they may be administering treatment or driving. Please leave a message with the patient’s name, your name, department/ward, contact number and brief details, and we will return your call as soon as possible
