Our Neurology Long Term Conditions (LTC) Team supports adults living with long‑term or acquired neurological conditions. We work with you and your families to manage symptoms, improve independence and achieve your personal goals.
We provide care for people with conditions such as:
- Acquired brain injury, including head injury
- Epilepsy
- Motor neurone disease (MND)
- Multiple sclerosis (MS)
- Parkinson's disease
- Spinal cord injury
Our team works closely with regional specialist centres, local health and social care partners, and voluntary organisations to ensure coordinated, holistic support.
Our team
Our multidisciplinary team includes specialist nurses, therapists and medical staff with expertise in managing neurological conditions. We also work with wider health and social care services to make sure you receive the right support at the right time.
Referrals
We accept referrals from any relevant health or social care professional, or, if you've been seen in our service before, you can self-refer.
All referrals are triaged by a clinician, and appointments are offered based on clinical need.
Where we are
We offer appointments in a range of settings depending on what is most suitable, including:
- Local health centres
- The therapy gym at Breightmet Health Centre
- Virtual appointments (telephone or video)
- Your home
The Fatigue Management Programme consists of seven short sessions designed to support people who are experiencing fatigue.
Fatigue can occur for many different reasons. If your therapist has recommended this course, you may have been diagnosed with ME/Chronic Fatigue Syndrome (ME/CFS) or have a neurological condition associated with fatigue.
The main aim of the programme is to help you understand and manage your fatigue, enabling you to regain a greater sense of control over your daily life. The sessions will introduce strategies such as pacing your activities, balancing activity with rest, and recognising the importance of getting enough rest.
This course should only be undertaken under the guidance of a clinician.
What do the seven sessions involve?
Each session is approximately 5–10 minutes long. You can work through the sessions at your own pace and pause or revisit them whenever needed.
