Service summary
Our Neighbourhood MDTs (Multi-Disciplinary Teams) is a forum where different professionals come together to discuss people who may need support from more than one service.
This could include nurses, social workers, physiotherapists, occupational therapists, pharmacists, GPs, mental health staff, housing teams, voluntary organisations and other specialists.
By bringing everyone together, the teams can share information, agree the best way to support the person, and make sure care is joined up rather than delivered separately by different services.
Bolton has six neighbourhood teams, with residents allocated to a team based on where they live. Referrals are made using a referral form and are reviewed at fortnightly MDT meetings. Each neighbourhood has six 15-minute discussion slots per meeting.
The goal is simple: to help people get the right support, from the right people, at the right time, as close to home as possible.
How to refer into this service
Inclusion criteria
- 18 years and over
- Palliative Care Patients
- Individuals whose presenting needs are causing concern to MDT members
- Frequent users of services
- Individuals who are identified as frequent attenders to Emergency Services
- Individuals who have complex presentations where interventions are problematic, have been unsuccessful and/or they are high risk
