Service summary
The Chairman's Fund provides financial assistance for essential items and practical interventions that remove barriers to safe hospital discharge, prevent avoidable hospital admissions and enable people to remain independent at home.
Support may include:
- Essential bed provision
- Mattresses
- White goods
- Small household items
- Key safes
- Minor adaptations
- Essential furniture
- Practical support where no statutory funding is available
Support is considered on an individual basis following assessment.
How to refer into this service
Inclusion criteria
Adults experiencing financial hardship where essential items will support hospital discharge, admission avoidance or independent living.
