Home from Hospital – Admission Avoidance (Home Support)
Service summary
Age UK Bolton's Admission Avoidance service provides rapid, short-term practical support to adults aged 50 plus at risk of an avoidable hospital admission. Working closely with health and social care partners, the service responds quickly to help people remain safe and independent within their own home.
Support may include welfare visits, shopping, prescription collection, light practical tasks, food provision where appropriate, home safety checks, signposting, wellbeing support and referrals into longer-term community services.
The service aims to prevent unnecessary hospital admissions, promote independence and improve wellbeing through early intervention.
