Service summary

This service only accepts referrals from within Bolton NHS Foundation Trust and Bolton Council

The Integrated Transfer of Care hub (ITOCH) supports the safe, timely and person-centred discharge of adults from hospital in line with the National Discharge to Assess (D2A) model and Home First principles. The service works collaboratively across health, social care and voluntary sector partners to ensure individuals receive the right support, in the right place, at the right time following discharge from hospital.

The team coordinates discharge planning, facilitates access to community-based support and assessment pathways, and promotes independence by ensuring ongoing care and support needs are assessed in an individual's usual place of residence wherever possible, to support effective discharge and reduce unnecessary delays.
