We support you from the moment you arrive in hospital following a stroke, through your recovery, and back into the community.
Our specialist stroke team will work with you and your family to assess your needs, plan your care and support your rehabilitation.
What to expect
In hospital
During the first few days your care will focus on:
- Assessing your symptoms and medical needs
- Preventing complications
- Beginning rehabilitation as soon as you are stable
Therapists will work with you to help you regain movement, communication and confidence. Our aim is to help you leave hospital as soon as you are medically ready.
Leaving hospital
Before you go home, we will agree a discharge plan with you. This will explain:
- The support you will receive
- Who will be visiting you
- When your therapy or follow‑up appointments will happen
Our community rehabilitation teams will continue your care once you are home.
Follow‑up care
You will have:
- A review at 6 weeks
- A review at 6 months
- Annual reviews arranged by your GP
Your GP will remain your first point of contact for any medical concerns, as well as ongoing monitoring such as blood pressure and cholesterol checks.
Where we are located
You will be cared for on the hospital's stroke unit.
Rehabilitation after discharge is provided at your home or in local clinics depending on what you need.
