2 Hour Urgent Community Response (UCR) including Falls Service

2 hour Urgent Community Response is the locality's rapid response admission avoidance service, delivered by the Admission Avoidance Team, providing assessment and intervention within two hours for people experiencing a health or social care crisis. The service supports a Home First approach by preventing unnecessary ED attendance and hospital admission through multidisciplinary assessment, treatment, falls response, social care intervention, and direct access to Hospital at Home and other community-based alternatives.

The team conduct holistic assessment of needs to promote independence, safety and confidence during an acute period of illness and reduce avoidable attendances/admissions to hospital.

Pathways / inclusion criteria:

• Chesty cough or respiratory symptoms
• Suspected urine infection
• Abdominal pain, constipation, diarrhoea, vomiting
• New rash
• Sudden change to mobility
• Sudden loss of appetite or fluid intake
• Sudden change in behaviour
• Sudden concern from staff which is not dealt with could lead to hospital admission
