2 Hour Urgent Community Response (UCR) including Falls Service

To provide a 2 hour urgent care response to adult people in their own homes or usual place of residence, which helps them to avoid hospital attendance/admissions and enable people to live independently for longer.

The Admission Avoidance Team (AAT) provides urgent assessment and intervention to Bolton residents over the age of 18 and if they are at risk of attendance/admission to hospital.

The team conduct a holistic assessment of needs to promote independence, safety and confidence during an acute period of illness and reduce avoidable attendances/admissions to hospital.

Pathways / inclusion criteria:

•       Chesty cough or respiratory symptoms

•       Suspected urine infection

•       Abdominal pain, constipation, diarrhoea, vomiting

•       New rash

•       Sudden change to mobility

•       Sudden loss of appetite or fluid intake

•       Sudden change in behaviour

•       Sudden concern from staff which is not dealt with could lead to hospital admission
