Age UK Bolton provides free practical, short-term support to adults aged 65+ returning home following discharge from hospital, helping people recover safely and regain independence.

Support includes:

  • Settling people safely back into their home
  • Welfare visits
  • Shopping
  • Prescription collection
  • Medication collection and delivery from local pharmacies and hospital pharmacy to avoid delayed discharge
  • Essential food provision where required
  • Home safety checks
  • Light practical support
  • Signposting and onward referrals
  • Access to longer-term Age UK Bolton services where appropriate

The service works alongside hospital discharge teams and community partners to reduce delayed discharge and prevent avoidable readmissions.

How to refer into this service

Inclusion criteria

Bolton residents age 65 +

Individuals requiring short-term discharge support.

Referrals are accepted from:

·         Hospital teams including wards

·         Community health professionals

·         GPs

·         Adult Social Care

·         North West Ambulance Service

·         VCSE organisations

·         Self and family referrals where appropriate.
