Community Specialist Palliative Care
Service summary
The Community Specialist Palliative Care Team (CSPCT) provides specialist assessment, advice, treatment planning, symptom management, and end-of-life care support for patients with complex palliative and supportive care needs within community settings. The service operates under the clinical leadership of a Consultant in Palliative Medicine and is delivered by Specialist Palliative Care Clinical Nurse Specialists (CNS), working as part of a multidisciplinary team.
The service supports patients in their own homes, care homes, community hospitals, and other community settings, ensuring high-quality, person-centred care and reducing avoidable hospital admissions.
Patients may be referred when presenting with:
- Complex symptom management needs.
- Uncontrolled pain.
- Advanced progressive disease.
- End-of-life care needs.
- Complex psychological distress.
- Difficult advance care planning decisions.
- Repeated hospital admissions related to palliative care needs.
- Complex family dynamics affecting care delivery.
