Frailty Hospital at Home (Virtual Ward)
Service summary
Hospital at Home (H@H) Frailty offers a safe and effective alternative to inpatient care for older adults experiencing acute exacerbations of frailty-related conditions. These services are delivered by multidisciplinary teams through a combination of in-person care and technology-enabled remote monitoring.
The Bolton Admission Avoidance Team (AAT) plays a pivotal role in delivering urgent community response and Hospital at Home services across the locality. Operating seven days a week, the team comprises consultant geriatricians, advanced clinical practitioners, clinical practitioners, nurses, allied health professionals, nursing associates and health care assistants who provide rapid interventions, falls response, care home support, and hospital-at-home services.
H@H Frailty can accept referrals from patients, relatives, carers, general practitioners, care homes, ambulance services, community services and hospitals.
How to refer into this service
Inclusion criteria:
Clinical frailty score (CFS) of 6 or greater
CFS 5 with geriatric syndromes (falls, delirium/dementia, polypharmacy , urinary incontinence)
