Homeless and Vulnerable Adult Team
Service summary
includes a Lead Advanced Clinical Practitioner (ACP), a trainee ACP, specialist nurses, Nurse Associates and HCAs.
We aim to support the health and wellbeing of clients across the whole of Bolton, physically, socially and emotionally and tackle inequalities.
Our services
- Drop-in clinic at Lever Chambers Health Centre, Monday/Wednesday/Friday 10am-2pm
- Point of Care Testing
- Advanced wound care/leg ulcer management
- Home visits/street kitchen
- Hospital In-reach to facilitate a safe and timely discharge - Homeless and Vulnerable Adult Nursing Team are not based in the hospital and provide hospital in reach on Mon/Wed/Fri mornings. If ward staff wish to discuss this patient outside if these times, please contact the team.

- Holistic assessments/Prescribing
- Advice, signposting and guidance
- Seasonal vaccination programme
- Collaboration with internal and external partner agencies including Diabetes Team, Hep C services, Rough Sleeper Drug and Alcohol Services, Bolton Housing Options and Greater Manchester Police

Duty To Refer Links
If the person is an inpatient and states they are homeless please contact the Homeless Nursing Team 01204 463417 Mon-Fri 8-4 or via homelessnursingteam@boltonft.nhs.uk. Staff should also complete a Duty to Refer at the earliest opportunity and not wait until the patient is medically optimised for discharge
