Service summary

This service only accepts referrals from within Bolton NHS Foundation Trust 

Palliative Care is provided for people who have advanced illness that cannot be cured. The focus of palliative care is on improving the quality of daily life and supporting family and carers.

Some people may experience more complex physical, psychological, spiritual and social problems and require additional support. When this is the case people will be referred to the Specialist Palliative Care Team.

The Specialist Palliative Care Senior Practitioner Emergency Department In-Reach service is provided to identify and manage unmet palliative care needs and identify people who are sick enough to die that are admitted to the Accident and Emergency Department. The Senior Clinical Practitioner proactively screens the Electronic Patient Record for patients in the Emergency Department for any patients with unmet specialist palliative care needs. Once assessed the patient will either be  treated and discharged home, admitted to hospital with care and support from the hospital Specialist Palliative Care Team, transferred to Bolton Hospice or admitted home with advanced interventions in the virtual ward environment.
