You must have parent/ carer consent before submitting a referral.

Referrals must give clear information about:

• What you and others have noticed
• How the child's difficulties affect their day‑to‑day life
• Any assessments already completed (e.g. WellComm)
• Relevant reports or plans (e.g. EHCP)

Early Help Assessments are no longer accepted as a referral route but can be included for information.

Dysphagia (feeding/swallowing) referrals

If concerns are ONLY about feeding or swallowing, please use the dysphagia-specific form and include:

• Medical history
• Early feeding history
• Current diet
• Reason for concern

These referrals should be marked urgent.
