Your GP or healthcare professional can refer you to this service if you’re newly diagnosed or need help to manage your condition.

We can help you manage:

• type 1 diabetes
• type 2 diabetes
• diabetes in pregnancy (gestational diabetes)
• diabetic complications e.g. kidney problems, foot ulcers and foot disease

We can also help you with:

• diabetes education
• advice about your diet and medication
• access to insulin pumps and other technology
• additional support before or during pregnancy for diabetic mothers

After 20 years at the Diabetes Centre, we’re making some changes to bring your care closer to you.

What’s happening?

We’re relocating our diabetes services to local health centres across Bolton.
You’ll still receive the same great service from the same great team, but in a location that’s easier to reach.

• Horwich Health and Wellbeing Hub, BL6 5PY

Most clinics are

• Easy to access
• On bus routes/ have transport links
• Free to park at (with the exception of Bolton One)

When are we moving?

We will be moving clinics gradually during October and November.

Need to know

Your clinic will depend on your current doctor and the type of diabetes care you need.

Once clinics are up and running, you’ll have the option to change doctor if it helps you attend a more convenient location.

But don’t worry, we’ll make sure you have all the details before your next appointment. You don’t need to contact us, but our phone number is staying the same, so if you have any questions, you’ll still contact us in the usual way on 01204 462400.

We are a consultant-led, specialist service that delivers your diabetes care at both the hospital and in the community at a number of health centres.

Our service offers a full assessment of people with complex diabetes related conditions including issues such as medication, diet, education, risk factors and complications.

Endocrinology is also a consultant-led specialist service delivering treatment for various endocrine problems such as thyroid, parathyroid, adrenal, pituitary diseases and others.

We have close links other centres such as Salford and Manchester to deliver the best possible care for patients.
