Neurology Long Term Conditions

Our Neurology Long Term Conditions (LTC) Team supports adults living with long‑term or acquired neurological conditions. We work with you and your families to manage symptoms, improve independence and achieve your personal goals.

We provide care for people with conditions such as:

  • Acquired brain injury, including head injury
  • Epilepsy
  • Motor neurone disease (MND)
  • Multiple sclerosis (MS)
  • Parkinson's disease
  • Spinal cord injury

Our team works closely with regional specialist centres, local health and social care partners, and voluntary organisations to ensure coordinated, holistic support.

Our team

Our multidisciplinary team includes specialist nurses, therapists and medical staff with expertise in managing neurological conditions. We also work with wider health and social care services to make sure you receive the right support at the right time.

Referrals

We accept referrals from any relevant health or social care professional, or, if you've been seen in our service before, you can self-refer.

All referrals are triaged by a clinician, and appointments are offered based on clinical need.

Where we are

We offer appointments in a range of settings depending on what is most suitable, including:

  • Local health centres
  • The therapy gym at Breightmet Health Centre
  • Virtual appointments (telephone or video)
  • Your home
