Service summary

We provide urgent assessment and intervention to Bolton residents over the age of 18.  The team conduct a holistic assessment of the patients’ needs to promote independence, safety and confidence during an acute period of illness.  Relevant referrals to key partner agencies will then be made to ensure continuity of care.

Our team is made up of over 50 members of staff, including:

  • Nurse Assessors
  • Physiotherapists
  • Occupational therapists
  • Mental health workers
  • Support workers
  • Social workers
  • Pharmacy technicians

We provide the following clinical services:

  • Assessment of Acute on the day conditions
  • Prescribing by a team of ACPs to support management at home
  • 30-day pathway to support early discharge and prevent readmission
  • Virtual Ward provision
  • 2-hour urgent community response
  • Urgent therapy assessments
  • Urgent equipment provision
  • Social care referral
  • Advice, signpost and guidance
  • Subcutaneous fluids
  • Oximetry at home – COVID pathway
  • Managing the deteriorating patients
  • IVAB (training in place)
  • Venepuncture
  • Post falls assessments

 

As a team we aim to:

  • Think home first - prioritising caring for patients in their usual place of residence
  • Manage a deteriorating patient safely at home – avoid hospital admission and improve patient experience
  • Support safe and timely discharge from hospital
  • Support other health care professionals and services, including GPs, NWAS, residential and care homes

How to refer into this service

Our Advanced Clinical Practitioners operate between 8am-8pm.
