Our Homeless and Vulnerable Adults Service supports people in Bolton who are experiencing homelessness, are vulnerably housed, or are living with complex lifestyles that affect their health and wellbeing. We provide accessible, compassionate healthcare to help people get the support they need at the right time.
Who we care for
We support adults who:
- Are sleeping rough, staying in hostels, temporary accommodation or unstable housing
- Are not registered with a GP
- Have health needs linked to homelessness, drug or alcohol use, or other vulnerabilities
How to access the service
You can access the service in several ways:
- Outreach – our team visits hostels, temporary accommodation and people sleeping on the streets
- In‑reach – we see patients admitted to the Royal Bolton Hospital
- Drop‑in clinics – available at Lever Chambers Health Centre
- Monday, Wednesday and Friday
- 10am – 2pm
- No appointment needed
We can help you register with a GP if you do not currently have one.
What to expect
At your appointment, we will:
- Carry out a health assessment based on your immediate needs
- Support you to access GP registration
- Help you connect with health, social care and housing services
- Offer ongoing support if you need additional help with complex health or social needs
Our team works closely with partners across Bolton to make sure you receive joined‑up care.
