Our neighbourhood approach

Moving care closer to the people that need it.

We are transforming the way we deliver health and care by working in neighbourhoods; small, local areas where NHS teams, GPs, social care, voluntary and community organisations work together to support the health and wellbeing of local people.

This makes support easier to access, closer to home and much more joined‑up between teams. It also means we can focus more on prevention and giving people help earlier, rather than waiting for problems to escalate.

Neighbourhood working means people can access the right care, in the right place, at the right time, often without needing to come to hospital.

Together, these teams:

  • proactively support people with long-term conditions
  • help people stay well at home
  • provide rapid community response
  • work with families, carers and communities
  • focus on prevention, independence and quality of life

Why we are moving to neighbourhood-based care

This shift reflects a wider movement across the NHS to:

  • support people closer to home
  • reduce avoidable hospital admissions
  • tackle inequalities at a local level
  • focus on the strengths of communities
  • deliver care that is proactive, not reactive

Each neighbourhood has a clear set of priorities based on local population health needs and is supported by shared data, planning and resources.

This focused support includes diabetes, cardiovascular disease and mental health – and is driven by the needs of the people who live in that area.

Together, we are building strong, resilient neighbourhoods where people can thrive.
