Our adult service includes therapists, assistants and admin staff based across Bolton.

• Royal Bolton Hospital (Acute Team)
• Lever Chambers (Community Team)
• Crompton Health Centre (Community Stroke Team)

Once referred into the service your referral will be assessed, and you may be offered:

• Face‑to‑face or telephone review
• Home visit
• Nursing or residential home visit
• Community clinic appointment

Support will depend on your patient's needs and the setting.
