Patients in Bolton reminded to update their details with GPs

Bolton NHS Foundation Trust is urging patients to review and update their contact information to ensure they receive important communications about their care.

As part of a commitment to providing high-quality services, the Trust wants to ensure that patients receive timely updates about appointments, test results, and treatment plans.

Patients are kindly requested to verify their contact details, including phone numbers, email addresses, and postal addresses, to ensure accuracy. This can be done by contacting their GP surgery and asking to update their contact details.

By keeping your contact information current, you will receive:

  • Appointment reminders and scheduling updates
  • Test results and diagnosis information
  • Treatment plan details and medication instructions
  • Important health advisories and recalls
  • Prescription reminders and repeat prescription requests

The Trust has an ongoing campaign for both staff and patients called ‘Know Your Patient’ to remind everyone about the importance of having and providing up-to-date information.

Patients are encouraged to keep their diversity details, such as disability status, ethnicity, religion, and sexual orientation, up to date. This helps ensure care is tailored to their individual needs and enables Bolton NHS Foundation Trust to better understand and meet the needs of the communities it serves.
