We request a minimum donation of £25, which includes the memorial plaque (engraved with your chosen wording), installation in the baby memorial garden at the Royal Bolton Hospital, and a short service led by a member of the Chaplaincy team (and interment of ashes) if required.

Your donation will also support the maintenance of the baby memorial garden and the annual Wave of Light event.

To order a memorial plaque, please complete the order form at the end of this information pack and send to the address stated within the form.

You can make your donation either by cheque or BACS transfer. Please do not use an online giving platform, such as JustGiving or Virgin Money Giving to make your donation.

Please see the order form for further details.

Please contact a member of the Chaplaincy team to discuss placing an order for a baby memorial plaque if your baby was born or cared for elsewhere.

No, it is personal preference. Some parents choose to order individual plaques while others opt for a shared plaque.

If you are ordering more than one plaque, please complete separate order forms for each. A single payment can be made, but please include the names on each plaque on the back of your cheque.

There is no character limit; however, please be aware that text size will be reduced to fit larger messages or verses, which may affect legibility.

Orders for plaques can be placed at any time; however, please note engraving and installation can take up to four weeks from the point of ordering.

If you would like your plaque installed before the baby memorial service in March or the Wave of Light service in October, please ensure we receive your order by 15 February and 15 September respectively.

We cannot guarantee the fulfilment of orders received after these dates.

Please indicate on your order form that you would like further information about a service or interment of ashes and a member of the Chaplaincy team will contact you to discuss these options with you.

Unfortunately, we cannot accept specific location requests for the installation of baby memorial plaques.

We respectfully ask families not to place non-perishable items and ornaments, such as soft toys, plastic and silk flowers or balloons in the baby memorial garden. Such items will be removed from the garden and kept for six months for families to reclaim.

Your co-operation in this matter is greatly appreciated and helps us to ensure the garden is kept tidy for all to enjoy.

Please contact a member of the Chaplaincy team to discuss any issues or concerns.

We will require details of your plaque, for example baby name and plaque unique reference number to help you with your enquiry.

As a registered charity (1050488) all monies received are donations and we are therefore not permitted to process refunds. This does not affect your statutory rights.
